Don’t have a passport and need to see the world? The HCPL Special Passport Acceptance Fair can get you there. You can apply in-person and meet with passport acceptance agents without needing an appointment. This event is for first-time applicants and children who must apply in-person using Application Form DS-11. Both parents of a minor applicant aged under 16 must be present. One parent must be present for applicants aged 16 and 17. Agents will also be available to schedule future appointments, review passport application documents and answer questions. WHAT TO BRING TO THE FAIR: 

Applicants must bring the following items to the fair:

  1. Bring completed DS-11 application form in black ink - DS-11 forms are available at the library or customers can complete and print the DS-11 ahead of time at travel.state.gov
  2. Bring evidence of U.S. citizenship ‑ One of the following original documents or official, certified copies of original documents. Photocopies will not be accepted.
  • Original or official, certified copy of U.S. birth certificate
  • Fully-valid, undamaged U.S. passport (may be expired)
  • Certificate of naturalization or citizenship
  • Consular Report of Birth Abroad
  1. Bring current photo ID with signature  One of the following forms of identification:
  • Valid, up-to-date driver’s license (not temporary or learner’s permit). Additional identification documents are required when presenting an out-of-state driver’s license.
  • Current, unexpired U.S. passport
  • Valid, up-to-date U.S. military identification card
  1. Bring separate payments for each application - Acceptable forms of payment include personal check, money order or cashier's check. Patrons may not pay with cash, cards or starter checks. Agent will staple payment to each application. Expedited services are available upon request but require an additional $60 fee for each application.
  • Payment to U.S. Department of State for each application (no cash or cards):  
  1. Adult passport book: $130 (Expedited: $190), Adult passport card: $30 (Expedited: $90)
  2. Minor passport book $100 (Expedited: $160), Minor passport card: $15 (Expedited: $75).
  • A $35 check or money order to Henderson County Public Library for each application.

 

  1. Passport photo - The library has a photo booth available for adults for $12.99. The booth accepts credit and debit cards.
  2. Bring a divorce decree if you resumed using your maiden name after your divorce.
  3. Additional documentation may be required.
